CITIZEN OF THE YEAR
City of Lynn Haven
ENTRY FORM INFORMATION
Submit nomination by December 31 of the current year.
Purpose: To recognize and award outstanding citizens of Lynn Haven who have provided his or her free time and services to promote the prosperity of Lynn Haven. To acknowledge those citizens that make a positive contributions and commitments to improve the community in which we live.
The Citizen of the Year is awarded to any person that has volunteered their time and services to support the community of Lynn Haven. Those efforts may include working hand and hand with City Boards and Civic Clubs to improve the quality of life of Lynn Haven. They may have volunteered their time toward City related matters, community involvement, and local youth or senior citizen programs.
To nominate someone for this award, please do the following:
1. Please make sure the nominee is a Lynn Haven resident.
2. Provide nominees name, address and telephone number.
3. Print a brief narrative describing the accomplishments or services rendered.
4. Please submit the nomination on or before the deadline to the Department of Leisure Services. 825 Ohio Ave. Lynn Haven, Fl. 32444
A nomination committee will select and recommend a winner. The winner will be announced in January.
